We will respond to any requests from individuals or their representatives to state the purpose of use of, disclose, correct the content of, add to, delete, stop using, or erase the disclosable personal information we possess, or to stop providing it to third parties (hereafter “requests for disclosure, etc.”) through this company’s designated application form and the following procedure.

    We will charge the following amount (which includes the fee for our reply) for each application only in the case of requests for notification of purpose of use and requests for disclosure. Please enclose a fixed amount postal money order in the following amount with your mailed application form.

  5. Means of Response to “Requests for Disclosure, Etc.”
    As a general rule, we will respond in writing (by sealed post) to the address of the concerned individual indicated on the application form. Please allow us a period equivalent to two weeks of this company’s business days to review your request after receiving your documents. We will inform you if we require more than two weeks.
    • Personal information accompanying a “request for disclosure, etc.” will be used within the scope required to respond to the “request for disclosure, etc.” only.
    • We may not be able to respond to “requests for disclosure, etc.” in the following cases. In such a case, we will inform you of our inability to respond and the reason.
      • We cannot confirm the identity of the concerned individual or representative
      • There is a flaw in the designated application documents
      • The object of the request for disclosure, etc. is not “disclosable personal information” (*)
      • Responding to the request risks harming the life, body, property, or other rights or interests of the concerned individual or a third party
      • Responding to the request risks severely obstructing the proper operation of this company’s business
      • Responding to the request would violate the law
